One URL per form. A submission becomes a sheet row or a warm-up email. Otto does not store it.

Settings → Forms is for admins and reviewers. New form URL asks for a name, then Google Sheet or Gmail. A sheet needs the Sheets connection to be able to write. Gmail needs a connected mailbox. Copy URL, and Activate or Deactivate, are on each form.
